All new employees must complete the online enrollment process on UKG within 30 days of their eligibility date.

Employees will need to login using their Employee ID as their username. Once logged in, click on the three blue stacked lines just left of the Lakeside/UKG logo > click on the single headed icon > My Benefits > Enrollment.
Employees and spouses must take action in 2021 to qualify for the 2022 HSA contribution from Lakeside.
- Employees hired by 2/28/2021, and their spouses, must complete a biometric screening, personal health profile (PHP), and the tobacco affidavit by 8/31/2021 to qualify for the 2022 HSA contribution from Lakeside.
- Employees hired between 3/1/2021 and 6/30/2021, and their spouses, must complete the Personal Health Profile (PHP) by 8/31/2021 to qualify for the 2022 HSA contribution from Lakeside ($1,000/employee and $500/spouse).
- Employees hired between 7/1/2021 and 11/30/2022 will automatically qualify for the 2022 HSA contribution. The contribution will be pro‐rated on a quarterly basis for employees hired after 1/1/2022.
